ATHLETIC HANDBOOK & FORMS
We are enthusiastic participants in the Florida High School Activities Association (FHSAA) competing interscholastically in the following sports: Football, Baseball, Cheerleading, Volleyball, Softball, Boys and Girls Basketball, Cross Country, Golf, Soccer, Tennis, Track and Swimming.
IMPORTANT REMINDER!
Any student wishing to participate in any St. John Neumann Athletic Program for the 2008-2009 school year, needs to have the following documents in to the Athletic Department as soon as possible:
- FHSAA Physical Form filled out by a physician. Physical Forms
- FHSAA Consent and Release Form filled out and signed by student and parent. Consent Form
- Parent's Code Form signed by both parent and student. Parents Code
- A copy of his/her Birth Certificate (if you turned in a copy of your Birth Certificate last year, there is no reason to send in a new copy). The Athletic Handbook is also available in the Athletic Assistant's Office between the hours of 9 am - 1 pm. Athletic-Handbook-09-10.pdf
